HIPAA Notice of Privacy Practices
Park Avenue Dental takes your privacy seriously. This notice describes how your medical information may be used and disclosed, and how you can access it. Please review it carefully. You may also read our Privacy Policy.
We are required by law to maintain the privacy of your Protected Health Information ("PHI"), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI.
How we may use and disclose your health information
- Treatment — sharing information with other healthcare professionals involved in your care.
- Payment — using information to bill and process claims with your insurance provider.
- Healthcare operations — for quality improvement, training, and administrative purposes.
We may also disclose information to family members or others involved in your care, unless you object, and in certain circumstances required or permitted by law (such as public health reporting, audits, court orders, or to prevent a serious threat to health or safety). PHI that is genetic information is never used for underwriting purposes. Any other use or disclosure requires your written authorization, which you may revoke at any time.
